Optional Claude model checks

Generate one optional evaluator from the current project configuration, using the installed plugin path:

Use --evaluator agent for an agent evaluator. The default, off, generates no model hooks. Each generated settings file selects one evaluator for Stop and SubagentStop; load one file per session. Regenerate it after changing Concise configuration or environment overrides.

The evaluator checks reply padding with the resolved rules, bypasses, soft-fail setting, and agent exemptions. It allows a reply when stop_hook_active is true. Prompt checks have a 15-second timeout; agent checks have 30 seconds. These checks use model judgment. They are absent from the shipped manifests. Codex skips prompt and agent handlers.
